内分泌替代疗法在临床实践中的应用

    PREFACE

 
Endocrine Replacement Therapy in Clinical Practice, an update of Hormone ReplacementTherapy published by Humana Press in 1999, aims to assist the endocrinologist,gynecologist, pediatrician, urologist, general surgeon, neurologist, neurosurgeon, psychiatrist,generalist, and trainee in management of their patients with hormonal deficienciesor altered hormonal synthesis or responses. Many new authors have added severalnew chapters, and all of the previous chapters have been updated. Endocrine testing usedto diagnose endocrine disorders and monitor hormone replacement therapy is reviewed.However, detailed discussion of physiology and pathophysiology is not an aim of thisbook, and these topics are covered in other volumes on endocrinology. Although no oneis considered an expert in all areas of endocrine replacement therapy, each of the authorshere have extensive knowledge and experience in the management of patients with specificendocrine disorders requiring hormone replacement treatment.
 
    Although some differences of opinion exist among experts in replacement therapy,each writer has attempted to give a balanced, unbiased recommendation. When comparableregimens exist, the authors have made this apparent to the reader who can thenchoose the best treatment for the individual patient. Cost, therapeutic effectiveness, androute of administration are all important considerations in making the final selection ofreplacement therapy.
 
    Endocrine Replacement Therapy in Clinical Practice is divided into seven main sectionscovering hormone replacement therapy of the pituitary, parathyroid and vitamin D,thyroid, diabetes, adrenal, gonads of men, and gonads of women. In Part I, the managementof diabetes insipidus and polydipsia in children is reviewed by Donald Zimmerman andGreg Uramoto and the treatment of diabetes insipidus in adults is covered by Gary Robertsonand Andjela Drincic. Darrell Wilson covers growth hormone therapy with emphasis ongrowth disorders of children, and George Merriam focuses on growth hormone and growthhormone secretagogues in adults.
 
    In Part II, issues related to treatment of disorders of calcium metabolism caused bydiseases affecting parathyroid hormone and vitamin D are presented by Eric Orwoll. InPart III, Steven Sherman and Luis Lopez-Penabad’s discussion on the use of thyroidstimulatinghormone in the diagnosis and management of thyroid diseases, includingthyroid cancer, provides invaluable perspectives. David Cooper reviews current guidelinesfor thyroid hormone replacement therapy in patients with primary and secondaryhypothyroidism. Special issues related to thyroid hormone replacement therapy in childrenwith congenital and acquired hypothyroidism are reviewed by Stephen LaFranchiand thyroid hormone replacement during pregnancy is discussed by Amy Sullivan andT. Flint Porter.
 
    Part IV includes six chapters discussing various aspects of hormone replacementtherapy of diabetes. Dana Hardin focuses on the diagnosis and management of diabetesin children, while Donald McClain reviews insulin and oral agents in the treatment oftype 2 diabetes. The special issues of management of diabetes during pregnancy arepresented by Kay McFarland, Laura Irwin, and Janice Bacon. Raymond Plodkowski andSteven Edelman report on their expertise using insulin pump therapy. In her chapter,Susan Braithwaite develops an algorithm for diabetic therapy during glucocorticoidtreatment of nonendocrine disease. The unique problems of treatment of the diabeticduring exercise are summarized by Robert Jones.
 
    Part V consists of three chapters. The first is by Lynnette Nieman and Kristina Rotherand covers glucocorticoid therapy for prenatal and postnatal life. Robert Dluhy summarizestherapy of mineralocorticoid deficiency disorders; and Syed Tariq, Hosam Kamel,and John Morley carefully analyze the pros and cons for dehydroepiandrosterone andpregnenolone therapy.
 
    Part VI has six chapters covering sex hormone replacement issues in men. A. WayneMeikle details issues relating to androgen replacement therapy in children and men withandrogen deficiency. The influence of androgen replacement therapy on sexual behavior,affect, and cognition is presented by Max Hirshkowitz, Claudia Orengo, and GlennCunningham. J. Lisa Tenover summarizes the merits and safety issues associated withtestosterone therapy in older men. Shalender Bhasin, Atam Singh, Keith Beck, andLinda Woodhouse present the rationale and recommendations for androgen therapyin men with muscle-wasting disorders, particularly as related to AIDS. ChristinaWang and Ronald S. Swerdloff discuss hormonal contraception, which is becomingmore commonly used in men and requires hormonal therapy to suppress spermatogenesiswhile maintaining normal testosterone concentrations. Peter Liu and DavidHandelsman contribute an article on the status of hormonal therapy of the infertile man.
 
    Part VII contains three chapters that focus on sex hormone replacement issues inwomen. Daren Watts, Howard Sharp, and C. Matthew Peterson make recommendationsfor hormone replacement therapy in women with primary and secondary hypogonadism.Androgen replacement therapy in women is still somewhat controversial, but is becomingmore accepted. Susan Davis and Henry Burger present a rational basis for androgentherapy in women. Shahar Kol focuses on hormonal therapy of the infertile women.
 
    Students, fellows, residents, generalists, and specialists in many disciplines will findEndocrine Replacement Therapy in Clinical Practice a highly practical reference for themanagement of patients. Patients will greatly benefit by receiving the best regimen formanaging their hormonal deficits.
